Dolezal and colleagues beautifully break down the mechanisms by which Shame exerts its effect on health outcomes. 1. Acute shame avoidance behaviours 2. Chronic shame health related behaviours 3. Stigma & social threats 4. Biological / physiological mechanisms . . A SHAME RESPONSE HAS A PHYSIOLOGICAL RESPONSE. . . Shame produces physiological changes in a number of body systems (you might notice, similar disruptions in health concerns such as..... PCOS, T2DM/IR, Cardiovascular disease etc)... . . Central Nervous System: Activates sympathetic-pituitary-adrenal medullary (SAM) axis and the hypothalamic-pituitary-adrenocortical (HPA) axis. . SAM = adrenaline/noradrenaline (think go-go juice). HPA = Cortisol, mobilise glucagon (fight or flight mode on). . . Immune system: Increase proinflammatory cytokines - TNF, IL-6, IL-1. . IL-6, associated with: muscle atrophy, slow muscle repair & accelerated wasting. Osteoporosis. Arthritis. Congestive heart failure. Prolonged or chronic infections. Production of CRP. . Posted @withregram @haesaustralia This coming week, from 28th Sept to 2nd Oct, is Weight Stigma Awareness Week. Each day, we will be posting about the many aspects of weight stigma which aim to increase our awareness about the many harms of weight stigma to raise awareness and offer ideas about how we can ALL make a difference in optimising care for all people, in all bodies. Before we launch into the week, we want to pay respect and tribute to the founder of Weight Stigma ...Awareness Week, Chevese Turner, a tireless advocate, colleague and friend to many of us. Chevese founded the Binge Eating Disorder (BEDA) in 2008 after identifying a need for greater advocacy for individuals affected by binge eating disorder (BED) and the providers who treat them. She speaks often about her own experiences of weight stigma, and the need for recognition of and access to care for those with eating disorders in all bodies. It is largely through the work of BEDA under Chevese’s leadership that Binge Eating Disorder was finally recognised as an official eating disorder diagnosis in 2013 and that since this time, people with BED have had greater access to care and funding streams. Chevese is a tenacious advocate and has helped to directly name one of the most under-acknowledged variables in health and wellbeing weight stigma. She also helped set in motion one of the most incredibly important articles about weight bias in health professionals when she encouraged Dr Rebecca Puhl, a weight bias and stigma researcher with the Rudd Center (Yale University), to survey eating disorder treatment professionals about weight bias. The results, although perhaps unsurprising, point to the work that lies ahead of us in healthcare to first address weight bias in healthcare providers to provide inclusive, respectful and high quality healthcare across all body sizes. We recognise that there is still a lot of work to be done, and HAES Australia are an advocacy organisation committed to changing the conversation. A reminder that you can’t see all chronic or acute illness on a body. Crohn’s disease affects so many parts of my life. Yet you can’t see it when you look at me. I will plan eating or not eating based on my symptoms and where I need to be. Sometimes the pain is so much I can’t stand straight. Other days I don’t even have a second of pain. My weight fluctuates greatly due to meds and symptoms yet people don’t realize when it drops that means I am not doing well yet I get compl...ements about how great I look. It is always on my mind as I plan vacations around public bathrooms and food choices and water. I plan around my 8 wk cycle of #stelera I get really fatigued and headaches for about 36hrs after injections. I feel a bit more symptomatic a few days before. This is life. This is life with an illness that never goes away. This is life with an illness many will never even know I have. So today I took my needle in the sunshine because I value every single day I have this disease because that means I am still alive. #crohnsdisease #crohnswarrior #crohnsandcolitiscanada See more

This past month I have been angry. All the time. About everything My anxiety is a struggle. My Crohn’s disease has been a struggle. Watching what has been happening around the world and trying to figure out where I need to make change has been hard but probably the most important I look at my kids and get angry for the world I have been complacent in creating and want to be part of changing for them. I get angry that I didn’t learn my country’s accurate history in school but then angry that I didn’t question it sooner. I have been angry a lot. It’s not helpful. I need to be part of change. Active change. I have read and I have listened but that is not enough. There needs to be action. Committed action. So I am looking for a new mindset for September. What would be your hashtag?? Posted @withregram @themindfuldietitian When we are planning, engaging in or delivering "health care" initiatives, programs or consultations, how much are we actually focussed on the humans we are serving? Or are we being distracted by "deliverables" & what is named "evidence-based practice" along with any demands to meet budgets, align with guidelines, protocols & more?? It's not uncommon that in our well-intentioned enthusiasm to want to "help," we forget the myriad of re...asons why so, so many people find this thing we call "healthcare" so difficult. Unless we are FIRST prioritising access, capacity, values, consent, priorities, lived experience and more, we can end up eliciting a powerful shame response in the people we are serving. What we best understand about shame is that it can drive experiences of stigma & marginalisation which in turn can lead to poor physical and mental health care outcomes. In regular person speak, what this means is that any conversations, messaging, education, campaigns or programs - however well intentioned - which lead people to feel badly about themselves is highly unlikely to lead to them making changes which may actually support enhanced wellbeing. AND (importantly), can cause additional harm. . . To be clear, it's not that health promotion, nutrition, mental health, sleep, self care etc should be thrown out. But when we rush in with our privilege to "deliver" to people and communities, then wonder why we are ineffective, this might be worth considering..... Are we asking people to first share their experience, their wisdom & their ideas before offering our own? Are we naming their experience for them, or inviting people to name their experiences for themselves? Are we first asking consent for all tests, procedures or being weighed/measured? Are we first considering people's capacity to actually participate in what we are delivering? Are we being culturally thoughtful? Are we examining our own privilege & biases? We all care. So let's actually put care first. #HAES #mindfuldietitian #care #inclusivecare

In a weight normative setting many of the above adverse outcomes are often framed as a result of higher weight with very little consideration given to the impact of weight cycling. .... . If we stay in the weight normative/centric frame for a moment - given what we know about intentional weight loss and weight cycling as it relates to the principles of non-maleficence and beneficence when is Treatment (weight manipulation) worse than the condition (high weight)? . .
