We have to stop relying on willpower. Willpower sucks. It's fleeting, unreliable and too dependent on external factors (stress, time, mood, energy, etc). So what does that leave us? Discipline. Habits. Work ethic. PLANS. . It's not enough to say "I'm going to workout 4 times this week". That'...s more like a wish. What's the plan? Did you schedule your workouts into your calendar? Did you tell your coach you're coming? Did you prepare your workout clothes, snacks you may need? Did you think about what barriers may come up to prevent you from attending and write down what you're going to do to overcome them? . It's not enough to say "I'm going to eat better". What's the plan? Did you log all the food you plan to eat the night before to evaluate your calories, macronutrients and micronutrients ahead of time? Did you do groceries on the weekend? Did you prepare easy-to-grab fruits and veg? Did you foresee upcoming events that are likely to throw you off track and create a plan of action? . Several studies demonstrated that phenolic compounds create higher levels of benefical microbes in the gut & can decrease microbes assoc. w/ obesity caused by diet. More research needs to be done, but given that we know that more fruits & veg consistently result in less chronic disease, current findings make sense! .... HIGH GLUCOSE/FRUCTOSE The Western Diet (WD) is high in glucose & fructose, which is suspected to negatively impact the gut. A mouse study found that mice fed high glucose or fructose diets had lower levels of good bacteria, higher levels of bad bacteria, 2.5-fold increased gut permeability (bad), & higher levels of inflammatory markers in the colon than mice fed a normal diet. Mice aren't humans, but we do know that high levels of sugar are assoc. w/ poorer health outcomes, so this makes sense too! . WESTERN DIET WD is high in total fat, animal proteins & refined sugars. It's been shown that high fat/high sugar diets increase bad bacterias & gut dysbiosis. Children eating a WD showed gut dysbiosis compared to those eating a diet high in plant based foods & low in lipis & animal protein . When the microbiota of US ppl were compared to those from Venezuela or Malawi, US had least microbial diversity (assoc. w/ poorer gut function). Animal protein appears to increase bile-tolerant microorganisms & decrease microbes that help metabolize plants (if not metabolized, we don't get benefits). Bile tolerant microbes can survive in the stomach & protect themselves w/ protective wall, making them resistant to antibiotics & our immune systems, causing potentially harmful health effects in humans. Studies suggest that additives found in WD cause dysbiosis, inflammation & alter the mucosal membrane of the intestines. That WD causes dysbiosis in the gut leading to obesity & metabolic disease is widely supported. . SUMMARY Eat real food, get a healthier gut! See more

Last #obesity post we talked about how excess fat causes dysfunction. Is this always the case? Maybe not. . Metabolically Healthy Obese (MHO) appear to avoid the complications of obesity (or of the Metabolically Unhealthy Obese, MUO) despite having the same BMI. It is debated whether this is a permanent state, or a "honeymoon phase" until dysfunction kicks in, but there do appear to be genetic differences between the 2 phenotypes. The known differences, among a few other th...ings, change the type of fat from subcutaneous (in MHO) to visceral & ectopic (in MUO), the latter of which seems to initiate the processes that cause dysfunction. http://ow.ly/YyWQ50BrOhI . So what can we do?! . High levels of free fatty acids in the blood can lead to insulin resistance (IR), a trait of MUO. Exercise (min 150 min per week) can lower these levels. You can also speak w/ your MD about pharmaceutical intervention. . The presence of intramuscular fat combined with low oxidative capacity (fitness) can lead to IR. Improving oxidative capacity (done via exercise ) appears to decrease IR (Pan et al. Diabetes 46: 983-988, 1997) . Weight loss, whether from diet or exercise, reduces Reactive Lipid Species (RLS) in muscle. RLS contribute to IR, mitochondrial damage & oxidative stress (MUO). So weight loss can decrease IR! (Dube et al Diabetologia 2011) . You are NOT doomed if you are obese. There are interventinons available that can improve your health & prevent disease progression. Ensure you have a trusted healthcare provider on your team. And remember, ENERGY EXPENDITURE PREDICTS PROGRESSION OF DISEASE MORE THAN OVERALL FITNESS! W/ obesity fat cells increase in # & size. Increased size leads to reduced blood flow & cell death. Cell death triggers macrophages that release inflammatory factors. These factors increase fatty acids (FA) & decrease adiponectin, leading to an inflammatory state. ... . Adiponectin stimulates AMPK (activates glucose & FA uptake) & metabolism of FA & triglycerides. It reduces glucose production in liver, increases glucose uptake, improves insulin sensitivity. Obesity often causes adiponectin RESISTANCE. . Leptin decreases appetite (the "full" signal) & decreases fat accumulation in non-fat cells. Important to keep fat from storing in dangerous places (i.e. viscerally/around organs). Leptin increases w/ increasing BMI causing leptin RESISTANCE. . Excess fat causes metabolic dysregulation in many ways (see images!); only 3 are highlighted here. Can we stop this dysfunction? YES. Last week we established #obesityisadisease . Now let's talk about its causes, because it is not always simply an excess of calories and/or a lack of movement. Personally we think lack of education is also a cause, which is why we strive to provide reliable health info. . The following info is summarized from "Let's talk about the causes of obesity" http://ow.ly/LS6x50B7yN2 . 1) Excess Calories. Notable reasons for our increased intake: snacking (food in addition to meals), ...take out food (high fat, high sugar), alcohol consumption. . 2) Empty Calories. The type of food we eat matters beyond calories. "Ultra-processed foods" might be especially to blame (chips, ice cream, white bread, candy, cake). These foods are created using fat, salt, etc to make our brain want more & more. They're also void of any nutrients so the body doesn't recognize it as real food and therefore doesn't realize its full, causing hyperphagia. . 3) Genetics. Evidence sugests #genetics contribute to 40 - 70% of obesity cases. Over 200 genes are strongly associated with obesity. This does not necessarily mean you are destined for #obesity, but it increases the likelihood. . 4) Obesogenic Environments. Social, economic, cultural & surroundings. Exs: huge portion sizes, targeted marketing & socioeconomic factors (ex Black and Latinx communities tend to have less acess to healthy food). . 5) Medications. Corticosteroids, SSRIs, anti-depressants, MAOIs, antipsychotics, hormonal birth control, insomnia meds, high BP meds, anti-seizure & epilepsy meds & even certain Type II diabetes meds are linked to weight gain or obesity. . 6) #Stress. Chronic stress increases cortisol, promoting fat accumulation. Cortisol is closely linked with excess weight. . 7) Poor #sleep. Ghrelin (a hunger-sginalling hormone) is increased with fatigue. Plus, if you're up later you have more time to eat! . These factors do not exist in isolation. The interplay makes #obesity complex. See more

Firstly, what is obesity? The Obesity Society defines it as an excess of body fat, where "excess" means an amount significant enough to reduce longevity and/or health (not a BMI higher than 30). . Secondly, what is a disease? A disease impairs normal functioning and has distinct signs and symptoms. . In 2013, after 3 decades of research, the American Medical Association named obesity a disease. YES. Obesity IS a disease. ... . The Obesity Medicine Association defines obesity as a chronic, relapsing, multi-factorial, neurobehavioral disease, wherein an increase in body fat promotes adipose tissue dysfunction and abnormal fat mass physical forces, resulting in adverse metabolic, biomechanical, and psychosocial health consequences. . Why is this significant? Calling obesity a disease means we have to recognize its complexities. Obesity is not always caused by inactivity & overeating. Obesity is seldom solved by eating less & moving more. Naming obesity a disease means that resources need to be put into finding solutions that work, because it is not a "behavioural dysfunction" as originally thought. . Plus, there is real problem in our healthcare system with bias towards those w/ obesity. Multiple studies have shown that patients w/ obesity do not receive proper care due to the biases/judgements of their healthcare providers. They have also shown that those w/ obesity avoid doctors b/c they often have such a negative experience at clincs. . Obesity is a COMPLEX issue. We can't become pain free, or injury resilient, if movements we repeatedly do (ex. brushing teeth, vaccuuming, bending over) are flawed. .... I often find that clients are great at implementing spine hygiene when they're dealing with an acute bout of pain; however, they drop the good habits once the pain subsides. Why is the rate of Type II Diabetes so high in South Asian adults & how can we effectively prevent disease onset? . This 2018 study analyzed skeletons as old as 11,000 years from varying regions in South Asia to identify whether the low lean mass in the South Asian population is a long term adaptation to ecological circumstance, or a more recent change brought on by dietary changes and the stresses of colonization. https://www.nature.com/articles/s41598-019-46960-9 .... The study concluded that low lean mass in South Asians was an ancient adaptation, probably in response to life in a hot climate. Driving the low lean mass may be a genetic variation that limits Myostatin - a gene responsible for muscle mass. . Why is low lean mass (ie low muscle mass) driving up Type II Diabetes rates? Muscle mass is a safe storage space for glucose; rather than it sitting in our blood and damaging our organs, glucose can saftely be stored as glycogen in the musle. During/after exercise, muscle has the ability to pull glucose out of the blood WITHOUT insulin An increase in non-lean mass (ie fat mass) damages the metabolic system because adipose (fat) tissue acts as its own organ, interfering with the signalling pathways of other organs and creating dysfunctional metabolic signalling . So do your genetics mean you're doomed? NO! Multiple studies have shown that lifestyle intervention can reduce the rate of Type II Diabetes development in South Asian adults. One meta-analysis found the risk to be reduced by 35% (doi: 10.1007/s00125-019-4905-2) . As an exercise scientist, my initial thought is to increase lean muscle mass! If we know that low lean mass increases risk of Type II Diabetes, then we can help clients/patients learn how to use resistance training as a proactive health strategy. . Many factors incl cultural, historical, socioeconomic, etc., will impact any community's success with health care. It is important for you, as the patient, to have discussions with your HCP that focus on what is realistic for YOU. What decreases your risk of disease/death more: aerobic training or strength training? .. .. .. ..... BOTH! Exercise is NON NEGOTIABLE (if you want to live a long, healthy, independent life). .. A study published in July of this year followed 479, 856 adults and divided them into 4 groups based on their self-reported physical activity: . 1) insufficient physical activity (not meeting National guidelines). 2) aerobic only. 3) strength only. 4) aerobic + strength. . At a median follow up of 8.75 years those who did only strength training OR only aerobic training had a significantly reduced risk of all cause mortality; an even larger reduced in risk was found in adults who engaged in both aerobic + strength training. . It is noteworthy that the same patterns were found when specific causes of death were evaluated, like cancer, cardiovascular disease and lower respiratory tract disease. . What are the guidelines?? We can't stress this enough!! If you return to poor movement patterns when your workout is over you are propelling yourself towards injury . We spend a large portion of the class - EVERY class - reviewing proper movement patterns so that your workout is safe. We remind everyone that patterns learned in class need to be applied outside of class. . TRX/Ring Rows = pulling a door open... . Squats = getting on/off couches, chairs, toilets & in/out of cars . Deadlifts = picking up laundry baskets, kids, soil . Hip Hinges = putting something in a low cupboard, laundry machine, washing your hands/face at the sink . Push Ups = mowing the lawn, pushing a stroller . Shoulder Press = changing a lightbulb, reaching to a high cupboard . And the phrase we say at least 100x per day "glutes tight, core braced, shoulders set!" should precede your movements. . You can relate all of your daily movements back to an exercise we do in class. Set yourself up the same way for both. . If you move correctly in class 2-6 hours per week, but incorrectly the other ~120 waking hrs you are breaking yourself down more than you are building yourself up. . Breakdown > Buildup = Injury. . Our ultimate goal is to create healthy, resilient humans. You know who you are. When you're on, you're ON. You workout 5-7x per week . You prep ALL the meals . Your health & fitness game is on point...when life allows. ....... When something comes up - a busy week at work, a familly member needs extra help, a busy weekend causes you to miss meal prep, you eat one "bad" meal, you miss one workout because you're not feeling well, etc - the "f**k it" approach takes over. ....... And when that "f**k it" approach kicks in, it i...s hard to turn it off. It can ruin your whole week, and suddenly a week turns into a month. All because you had one or two off moments. ....... "Success is the sum of small efforts, repeated day in and day out." We say that a lot here at CFB. Success is not made in one day, nor is it broken in one day. Or one meal. Or one missed workout. Or one off week. Success is the SUM of your efforts. You just have to ensure the majority of your efforts move you towards your goals. ....... I've seen the "all or nothing" approach sabotage a lot of hard work. Life is messy. Life is ultimately unpredictable. If we don't learn how to be flexible in our methods, success is much more difficult. ....... So the next time you get thrown off your game, roll with it. Be flexible with your schedule. ...During a pandemic. . I want to preface this post by stating it is not a push for toxic positivity. You are allowed to have hard moments & feelings. Life events that have minimal "silver linings" are going to occur. I'm talking about getting through the day to day here. And I'm talking about giving yourself a fighting chance. . Event + Our Response to Event = STRESS LEVEL. This is stress mediation. The term "stress management" implies we have limited power against th...e stresses thrown at us. In "stress mediation" we have control over 50% of the eqation. . I HATE when ppl tell me not to worry about the things I can't control; why would I worry about the things I CAN control? A happy medium exists; we have to focus on the controllable aspects of of an uncontrollable situation. . We can't control a global pandemic. But we can control what we do with our time. We can control our daily routine so that we give ourselves the chance to be mentally & physically okay. . From the W.H.O.: *Have a Routine!!* 1. Get up & go to sleep at similar times each day 2. Keep up w/ personal hygiene 3. Eat healthy meals at regular times 4. Exercise regularly 5. Allocate time for working AND time for resting 6. Make time for doing things you enjoy. BUT what if a mental health condition is being exasperated by the pandemic? 1. The same advice applies - creating a routine will help regulate mental health 2. If you were receiving treatment (medication, therapy, etc) continue it throughout the pandemic. 3. If you are too depressed or anxious to create these routines (even with the push from friends & family) TELL SOMEONE. There are resources available to help you. . If you see someone struggling, TALK TO THEM. Don't wait for them to come to you. Help ppl you love find the support they need. . This advice isn't easy. It's work. But we know that you are worth the effort it takes. And we are here to help if you need support.
