Hi all, just a quick update about what we are doing at Willow Family Medicine - YEG during COVID in regards to our Breastfeeding Medicine and Infant Nutrition Clinic. We are seeing our referrals escalate as I am sure other clinics and private pay options are closing. Further, patients have reported breast milk substitute shortages. Breastfeeding medicine and infant nutrition is an essential service and we will continue to work tirelessly to provide help to families. As such,... this clinic will continue to operate. We've tried telehealth with dyads but we just can't do the work properly, especially given the complexity of our patients. There is just too much in person medicine/assessment required. As such, we are asking all dyads to come to clinic (unless telehealth follow up can be arranged as appropriate). However, we are taking the following precautions; -We've discontinued almost all family medicine patients in our office. All converted to telemedicine. That means the only patients in our office are breastfeeding/infant nutrition patients (dyads). -Dyads are asked to leave support persons and other children at home. If they present to clinic with another adult, they are being asked to wait in the car. -All patients with respiratory symptoms are asked to reschedule. -Julia, our lactation consultant, and myself are taking all measures to ensure we socially distance. we have limited almost all non essential social contact. Work is our main focus. -We have deployed scrubs and aggressive sensitization in our office. We are diligent about what comes in and goes out in terms of possible COVID sources. We personally clean the rooms between patients. -No allied health are in our office at this time. Our admin person is are coming in after hours or working from home. -There is no more than 2 families in the clinic at one time. One is often packing up as the other is leaving. They are more than 2 m apart. -Frenectomies continue to be preformed. Our private pay clinic has been discontinued as our dedicated Nurse Practitioner has been called to work elsewhere during the pandemic. -We are down a doctor due to her increased work load in obstetrics and primary care at present. However, Julia and I have extended our hours and added clinic days to help see above average volumes. Truly, we are just working around the clock. -Given the small size of our clinic and separation from hospital, I do believe it is a safer option for families than visiting a hospital based clinic setting like the Misrecordia or Grey Nuns for support. We will do our best to accommodate as such. -Our office has all necessary PPE and cleaning products. -All patients are being informed of these precautions/changes before their appointment to help alleviate worries. Tonight, the reality of COVID coupled with the subversive privatization of primary care by our government in the form of TELUS Babylon Health (coincidentally connected to Shandros wife), encouraging patients to seek primary care for a fee rather than visit their family doctor has proven to be too much. I broke down. I pride myself of being strong for patients and my colleagues, but this is too much. Its the last straw at decimating socialized medicine. . Medicine is so much... more than a quick screen share and a prescription. I am angry at our government for proposing this ludicrous solution during a pandemic, exploiting the fears of patients and resource constraints. I am also angry at all the doctors who have done a terrible job for too long, therefore, making this look like a viable alternative for Albertans. . If our government wont pay doctors more than $20.00 a phone call, I cant keep my clinic running or pay my staff. If we (and many other clinics) close, patients are going to be forced to pay for primary care services. This is the true agenda of our government. The only other option for many of us, is to make patients come in for a visit (during a pandemic) so we can earn $38.03 instead. The government has threatened to audit us if we dont obey. . I cant ethically do that. I won't. I refuse. I took an oath to do no harm. But I dont know what else to do? My hand is being forced to do not only bad, but dangerous medicine. . Its rare that I truly dont know what to do. I am the fixer. I am the micropractice wizard. But I dont anymore. I dont know how to be a good doctor, business person AND risk my health and sanity for a system who refuses to acknowledge my worth, or uphold my patients right to universal health care. . I try to be funny. I try to keep it light. Patients need hope. But these dark times have gotten darker tonight and its our governments fault. . Please never vote for this tyranny again.
