We are 'Reaching' out to you with a WSC environment analysis. Let's break down the environment here. So it's the first snowfall of the season and early in the morning. The snow is accumulated and wet enough that it covers your boots. The ground has not had a chance to freeze yet so its muddy and soft. We are installing Tyvex from extension ladders. ......... Now we are standing on the ladder, working from the ladder up to 25 feet high, stapling the Tyvek to the wal...l. ......... Let's take a look at a few of the unnecessary risks here. Workers have taken the time to set up the ladders at the correct angle, are both wearing the proper ppe and the working area looks well set up. However, when working from a ladder at a height of 8 feet or more. Measured from the bottom of your feet to the furthest potential surface you would fall to then you must maintain 3 points of contact at all times in order to not be required to tie off. Leaning out to do work outside your reachable work surface is dangerous enough but choosing to remove a foot as well as a hand and then lean out while 10 + feet in the air is asking for broken ribs, impaled lungs a hemothorax or worse. ......... Take the time to consider the risks and evaluate the worst case scenario. The control measures that could have been taken wouldn't have costed money or any significant amount of additional time. Being aware of your surroundings, the environment, the people around you, the tools / equipment you have access to and pre-job planning are sometimes the difference between going home or not. Most of the companies I work with have extensive safety practices and procedures and yet more often then not we do not use the provided solutions. As you watch this video see how many risks these workers are taking for zero reason. All the tools are available to do the job safety and... yet laziness / rushing to get things done takes precedent once again. . . . Worker choosing to access a column via a ladder that is too short and is a step ladder being used as a single or extension ladder. Stepped off the step ladder and proceeded to stay untied at heights regardless of rebar anchorage completely accessible. Workers in the back are moving a large rebar column which is too heavy and seemingly unplanned as the worker who was working off the ladder and a second worker to the left of the video have to rush over to assist once the column starts to topple. So I walked to the end of the driveway and noticed the switch from the transformer to the house was blown open. Called Hydro One to report an outage and when they came they pulled a squirrel off the transformer This squirrel was blown apart and also had its tail blow clean off its body.
