On April 1, 2021, the Erie St. Clair LHIN will begin operating under a new name, Home and Community Care Support Services Erie St. Clair, with a focused mandate to ensure the ongoing stability of patient care, including home and community care and long-term care home placement. Health system planning and funding functions will also transfer to Ontario Health, the agency responsible for ensuring Ontarians continue receiving high-quality health care services where and when they need them. We are pleased to introduce Julia Faye Dyer, Indigenous Patient Manager (Anishinaabe eyaawid eniigaanzid) for our local health care system. Using culturally and... spiritually-relevant care, Julia provides support and advocacy for local Indigenous patients, caregivers, and their families; helping to navigate from hospital to community health services. Her work also focuses on health equity and social determinants of health, specific to Indigenous people and communities. Julia is from Chippewa of the Thames First Nation. Her Grandfather, Ken Albert, was a residential school survivor and served as Chief. As a Registered Nurse of 17 years, Julia’s training and work background include diabetes education, pre and postnatal education, with experience in developing and delivering training programs to staff, community organizations and medical providers on a range of health management topics. Additionally, she is trained in mental health and addictions, as well as crisis intervention. Julia works for the Erie St. Clair Local Health Integration Network and is stationed within CKHA’s Chatham site. She looks forward to working with the Indigenous community in improving health care experiences.
