Now that our local COVID numbers seem to be more manageable , we are expanding the hours for our vaccination clinic on Wednesdays. As we are moving into red zone on Monday, our clinic will have reduced hours and the acute care clinic will operate virtually . We have limited PPE supplies and the physicians will be unable to provide in-person care for the acute care clinic. We will, however, have an RN on site one day to administer vaccines if required. If you are comfortable doing so, please email us with the reason of the visit so that we can determine which day will suit your needs better. We are excited to announce that we will finally be having flu shot and immunization clinics! The first clinic will be from 11AM-2 PM on Tuesday November 3rd. We will also have another on Wed from 4-6 PM. These appointment must be pre-booked and will be accessible for children and adults. We also expect to expand our hours in the near future as we have new team members joining us.! Thank you so much for your patience and understanding during this busy period of transition !

Quick update ! We have a new online messaging system that allows us to securely send forms to patients prior to appointments. The forms help us to efficiently update chart information including medications , allergies and other important health information. It also allows us to send COVID screening questionnaires prior to visits as well as consent forms .... Forms will be emailed to you so PLEASE send us your email address before your appointment. You might also receive a text from one of our Medical office assistants requesting your email if we do not have it on chart . Thank you for your understanding Komoka Medical Team

Have you travelled outside of Canada in the past 14 days? Yes No 3. Have you had close contact with a confirmed or probable case of COVID-19? Yes No Results of Screening Questions: If the individual answers NO to all questions from 1 through 3, they have passed and can enter the workplace. If the individual answers YES to any questions from 1 through 3, they have not passed and should be advised that they should not enter the workplace (including any outdoor, or partially outdoor, workplaces). They should go home to self-isolate immediately and contact their health care provider or Telehealth Ontario (1 866-797-0000)to find out if they need a COVID-19 test.

It has been a period of transition for us and due to COVID constraints , we have had very little administrative support to answer calls and emails. Changes effective October 5th to London Middlesex COVID-19 Assessment Centres: Effective October 5th, operations at the London Assessment Centres are changing as they strive to meet the testing needs within our community.... What's changing? Carling Heights Assessment Centre ALL walk-in COVID-19 testing will be done at Carling Heights Optimist Community Centre, 656 Elizabeth St. The time card system will continue at this location. New hours of operation as of October 5: 9:00 am-7:00 pm, 7 days a week. Patients prioritized at this location: Have symptoms of COVID-19 Have been notified by Public Health or the COVID-19 App that they have been exposed to a confirmed case of the virus Live or work in a setting that has a COVID-19 outbreak as identified by Public Health Are eligible for testing as part of a targeted testing initiative as determined by the Ministry of Health or Ministry of Long Term Care Oakridge Assessment Centre Effective October 5, Oakridge will shift to booked appointments ONLY. The time card system will be discontinued at this location. Number for public to call: 519-667-6886 and follow the prompt. They will be asked to leave name, call-back number and reason for test. The Centre is committing to calling everyone back within two business days. Oakridge is prioritizing testing of: Symptomatic children ages 12 and under Symptomatic essential health care workers Asymptomatic individuals - limited to people who are: Residents, workers, visitors or caregivers to a retirement home or long-term care facility International students who have passed their 14-day quarantine period Farm workers Indigenous communities Other setting specific populations as defined by the Chief Medical Officer of Health (pre-operative or pre-chemo)
