**** EDIT: Deadline extended to March 22, 2021 **** COVID-19 Vaccination Clinics for Off Reserve Aboriginal People The New Brunswick Aboriginal Peoples Council and The Public Health New Brunswick are working together to offer/open COVID-19 vaccination clinics across New Brunswick for the Off-Reserve Aboriginal People. ... Public Health New Brunswick is working to allocate the vaccines produced by Pfizer/BioNTech and Moderna to said clinics. In order to have sufficient quantities of vaccines, we need to provide Public Health with the numbers of people who are interested in getting vaccinated by the end of March 2021. Any Aboriginal person (and spouses) living off-reserve, 16 years of age or older who would like to get the COVID-19 pandemic vaccine, please fill out this online form no later than March 22, 2021. Kindly note that the only way to get your name on the list to get vaccinated is through this online form: https://nbapc.org/covid-19-vaccine-online-registration-form If you know someone who might need help in filling this online form, please offer your assistance. Our office will not be able to offer assistance to fill out the form over the phone. We will not accept any forms coming in via email or fax. The way this online form works, is so we can export all the info filled into an excel sheet and share it with Public Health to prepare for the clinics. Once the clinics are set, we will be emailing all the people who filled out the forms, post it on our website: www.nbapc.org and post of on our Facebook page: https://www.facebook.com/NBAPC The personal information collected will only be used by the staff of NBAPC and Public Health New Brunswick at the time of the collection in order to provide the accurate number of vaccines, needles etc. at each clinic. We will ask you to provide consent for us to share your info with Public Health NB. Please make sure to read the Information Sheet about Pfizer-BioNTech:https://www2.gnb.ca//covid-19/information-sheet-vaccine.pdf and Moderna COVID-19 Vaccines https://www2.gnb.ca//covid/moderna_information-sheet_1.pdf. We also encourage you to fill out the consent form https://www2.gnb.ca//Pr/covid-19/COVID-19-consent-form.pdf print it and present it at the clinic. You can always fill out a consent form at the clinic the day of your vaccination, as well as ask any question you have to the nurses at the clinic.
